Please note: If you wish to amend the date of play of your booking you must cancel the booking using the Cancel Bowls Booking page and make a new booking via the Bowling Online Booking page.

This form is only to be used if you wish to amend the name of the guest player for your booking. We must have an up-to-date record of those that are playing.
